Whether you’re visiting Capital Surgeons Group for a surgical consultation, follow-up appointment, or procedure, we’re committed to making your experience as smooth and convenient as possible. This page provides important information to help you prepare for your visit, complete required forms, request medical records, understand insurance and payment options, and access additional patient resources.

We encourage all patients to review this information before their appointment. Completing forms in advance, bringing the necessary documents, and understanding what to expect can help ensure a more efficient visit and allow our surgical team to focus on providing the highest quality care.

Please call the Capital Surgeons Group office you would like to visit or the office of the surgeon you would like to see. Review each surgeon’s bio page for location information, as many of our surgeons see patients at multiple offices throughout the Austin area.

Our scheduling team can help you find the most convenient appointment location and answer any questions before your visit.

Telemedicine is available when making your appointment for most appointments.

Please arrive at least 15 minutes before your scheduled appointment and bring the following items to help ensure a smooth check-in process:

• Government-issued photo identification (such as a driver’s license)
• Current insurance card

To help your surgeon better understand your condition, please also bring any relevant medical information you may have, including:

• Test results
• X-rays
• Mammograms
• Ultrasound
• Other clinical records from your physician, specialist, or emergency room visit

You can save time by completing your New Patient Forms before your appointment.

If you need to cancel or reschedule your appointment, we kindly request at least 24 hours’ notice whenever possible. This allows us to accommodate other patients who may be waiting for an appointment and helps us provide timely care to everyone.

Capital Surgeons Group will keep your primary care physician and referring provider informed about your condition and treatment throughout your care.
